Looking for Poem & Author

My girlfriend heard a poem of all places on a TV commercial. I dont remember the words, but one of the lines is "near or far and nobody knows it but me". She really loved the poem so I am trying to find it for her. If anyone has an idea I would greatly appreciate any feedback.
