Rudyard Kipling wrote the lines,

The tumult and the shouting dies
The Captains and the Kings depart
Still stands Thine Ancient Sacrifice
An humble and a contrite heart

Lord God of Hosts be with us yet
Lest we forget, Lest we forget.

And I was wondering where these lines came from because I have not been able to locate them in a specific poem or piece of work. I would appreciate any information pertaining to these lines.
